School Based Wellness Centers

The LHC is in the business to provide school based wellness centers to the communities. We provide preventative, holistic, and comprehensive healthcare solutions to elementary children. We recognize that students do better in academics when they have the support they need for behavioral and/or mental health as well as access to physical and health education.

Our services are conveniently provided in our school-based wellness centers located within the schools and offers solutions to many of the health barriers that children may not otherwise receive.

We believe that by providing these services to students they have a chance to excel in school and lead a more productive life. Having our centers based in the schools help families to overcome barriers to receiving care such as transportation, appointment conflicts, and out of pocket cost.

Partnerships

LHC is interested in partnering with stakeholders to maximize student educational potential. LHC is in partnership with the Christina School District and is a state-recognized provider of School-Based Wellness Centers by the Department of Health Social Services Division of Public Health.

Diagnoses

The wellness center provides diagnoses and treatment and works with Primary Care Physicians to provide observations as well as receive direction for treatment when needed. The wellness center does not provide imaging, complex lab test, or hospitalization. The wellness center works in conjunction with the school nurse for an Emergency Action Plan prescribed by the student’s PCP for Medical Conditions such as diabetes, asthma, and seizures, or any chronic condition.

LHC-SBWC may receive referrals from school administration, PCP, or parent/guardian. Students must be enrolled by a parent or a legal guardian to sign a consent form. Parents must also submit a registration, release, and a Health Assessment History form. The completed forms must be received by the Wellness Center before any services will be provided.

The LHC-SBWC adheres to all federal regulations governing and abides by HIPPA laws.

School Based Wellness Center Enrollment

The Life Health Center (LHC) offers professional services through our school-based wellness center. These services include, Pediatric, Behavioral and Mental Health which includes Prevention, and Social Services. LHC has been providing a high caliber care to anyone in Wilmington, New Castle, Newark and surrounding area since its inception. Services have been provided at outpatient locations to whole families in an integrated health delivery system that focuses on the whole person and outcomes. All these services are now available through LHC in the Christina School District in New Castle County of the state of Delaware.

The goal of providing Behavioral Health and Preventive services through LHC directly at your child’s school is to ensure that support services are in place to assist in the reduction of:

  • Classroom disruptions/off task behaviors
  • Suspensions/non-compliance
  • Low self-esteem/poor self-image

You or your child’s school staff may refer your student for services at any time and if you consent to these support services for your child, you simply need to complete the attached forms and return them to the main office at your child’s school. Services through LHC are provided at no out of pocket expense to you as services are covered through accepted insurance plans.

Poor health outcomes from heart disease, diabetes, cancer, obesity and other disease areas can be conquered by paying attention to whole person health. LHC goes beyond physical conditions and promotes good emotional and mental health as a very important part to overall better health. Behavioral Health Counseling services are geared towards identifying barriers to success and providing tools to reduce and eliminate health issues.
